What is a Breast Abscess?

A breast abscess is a localized infection in the breast that results in a collection of pus. It often occurs during breastfeeding (lactational abscess) but can also affect non-lactating women (rare). Prompt diagnosis and treatment are essential to avoid complications and ensure a smooth recovery.

Causes and Risk Factors

Breast abscesses typically develop from untreated mastitis (inflammation of the breast), especially in breastfeeding women. Common causes and risk factors include:

  • Bacterial infection (commonly Staphylococcus aureus)
  • Cracked or sore nipples
  • Poor breastfeeding technique or milk stasis (if not expressed out)
  • Weakened immune system
  • Smoking or diabetes (in non-lactational abscesses)

Signs and Symptoms

Recognizing the early signs of a breast abscess is crucial for timely treatment. Symptoms may include:

  • Localized swelling or lump in the breast
  • Redness, warmth, and tenderness of the skin
  • Pain that may worsen over time
  • Fever or chills
  • Fatigue or feeling unwell
  • Pus or discharge from the nipple

Diagnosis and Evaluation

Diagnosis is done with a combination of clinical evaluation and imaging. Diagnostic steps include:

  • Physical Examination to assess pain, swelling, warmth and fluctuation
  • Ultrasound Imaging to locate the abscess and determine its size
  • Aspiration of fluid (if needed) for culture to identify the bacteria

Treatment Options

1. Antibiotic Therapy

Initial treatment often involves antibiotics to control infection. This is typically prescribed when the abscess is small or in its early stages when the fluctuation did not set in. 

Drugs: 3rd generation Cephalosporins, Amoxicillin.

2. Drainage Procedures

When an abscess does not respond to antibiotics alone, drainage becomes necessary:

  • Surgical Drainage: Recommended for larger abscesses or mastitis not resolving with antibiotics in 48 hrs.
  •  A small incision is made to drain the pus completely with a counter incision and drain placement when required. 

Post-Treatment Care and Recovery

Following treatment, we provide detailed aftercare instructions to promote healing and prevent recurrence:

  • Proper wound care – daily dressings
  • Follow-up imaging (if required)
  • Breast supports to relieve pain 
  • Support for breastfeeding mothers, including lactation consultation
  • Milk from affected breast should be removed manually or using breast pump (to prevent stasis of milk)
  • Guidance on breast hygiene, nipple care and self-monitoring

Most patients recover fully within a few days to a couple of weeks with the right treatment.

To reduce the risk of developing a breast abscess:

  • Practice good breastfeeding techniques and empty the breasts regularly
  • Treat mastitis early  
  • Keep nipples clean and dry
  • Avoid smoking
  • Maintain a healthy immune system
