Phimosis Treatment

Introduction

Phimosis is a medical condition where the foreskin of the penis cannot be retracted over the glans (head) of the penis. This condition is common in young children but can persist into adulthood or develop later due to infection, inflammation, or scarring.

Causes

  • Poor hygiene leading to accumulation of “Smegma” – foul smelling, cheesy like accumulation around the genitals
  • Recurrent infections such as balanitis: infection of head of penis (glans)
  • Chronic inflammation: long standing infections
  • Congenital – since birth (in some cases)
  • Uncontrolled Diabetes – causing recurrent infections/ Urinary infections
  • STDs: Sexually Transmitted Diseases

Symptoms

Common symptoms of phimosis include

  • Pain during urination or erections
  • Redness
  • Swelling of foreskin
  • Difficulty cleaning under the foreskin
  • Repeated infections
  • Ballooning of the foreskin during urination – in severe cases or in children

Diagnosis

Diagnosis typically involves a physical examination assessing the tightness of the foreskin and look for signs of infection or scarring. No specialized tests are usually needed.

Treatment

Treatment for phimosis depends on its severity.

Non-surgical:

  • Gentle daily stretching exercises
  • Avoid frequent infections by maintaining hygiene and strict control of sugars

Surgical management:

Surgical intervention is the ideal treatment in case of repeated symptoms or failed conservative management. Circumcision, the complete removal of the foreskin, is a definitive solution. It can be done under local anesthesia in open technique or stapler method.

An alternative is Preputioplasty, a procedure that loosens the foreskin without removing it, preserving more natural function and appearance.

Recovery

Post-surgery care involves maintaining proper hygiene, managing pain with prescribed medications, and ensuring adequate rest and avoiding frictional rub at surgical site. Recovery usually takes about one to two weeks, with gradual resumption of normal activities. Sexual activity can be resumed after 6-8 weeks once it heals fully with no discomfort.

Conclusion

Untreated phimosis can lead to discomfort, infections, and hygiene difficulties. Early diagnosis and appropriate treatment can prevent complications.
