What is a Phyllodes Tumor?

A Phyllodes Tumor is a rare type of breast tumor that develops in the connective (stromal) tissue of the breast. Unlike more common breast lumps, phyllodes tumors can grow rapidly and vary in their potential to become malignant (cancerous), around 10-15%. When malignant can spread to lungs or bones. 

Features :

Phyllodes tumors often occurs in middle aged or elderly. Presents as a painless, fast-growing lump in the breast. Symptoms may include:

  • A smooth, firm, and movable breast mass
  • Rapid increase in size over a few weeks or months
  • Stretching or visible changes in the skin over the lump
  • In rare cases, ulceration or nipple discharge

Due to their similarity to fibroadenomas, proper evaluation is essential.

Treatment Options

The primary treatment for phyllodes tumors is surgical removal. The type of surgery depends on the size, location, and type of tumor.

1. Wide Local Excision

  • Removal of the tumor with a margin of healthy tissue
  • Minimizes the risk of recurrence
  • Suitable for most benign or borderline tumors

2. Mastectomy: 

  • Removal of entire breast tissue along with the lump
  • Required for very large or malignant tumors where breast-conserving surgery is not feasible (when size of lump is bigger and no breast tissue left to maintain contour)
  • Rarely involves lymph node removal as spread is uncommon

Do Phyllodes Tumors Return?

Yes, phyllodes tumors have a risk of recurrence, especially if not removed with clear margins. Malignant tumors have a higher risk of returning. Follow-up care is essential to monitor for recurrence.
